Create Your Own CDR Report!

The online CDR Team Report Template was developed by ICAN/NCFR to assist you in creating your Team's CDR Report. As you go through the Template, you will be prompted to enter information regarding your CDR Team and its work. This process is much like taking an online survey or questionnaire. When completed, you will receive a fully-formatted CDR Report in PDF. The report is tied to your email address for retrieval. Once the report is completed and sent to you, it will be kept on our server for 60 days, after which it will be deleted.
